Telehealth Amendment​ (Effective November 12, 2023)​, this amendment adds telehealth as a permanent service delivery option for the following waiver services: Case Management; Community Transition Service; Comprehensive Care Management; Family/Caregiver Training; and Transitional Case Management.

HCBA Waiver Agencies

Most service areas are identified by county and include all of the zip codes within the county. However, individuals residing in Los Angeles and Orange Counties should refer to the links below to identify the Waiver Agency that covers their zip code.

Recent Change in Waiv​​​​​er Agency County Assignments:

Several counties have new Waiver Agency Assignments as of mid-2024. Alpine, Inyo, Marin, Mono, Napa, Inyo, San Francisco and San Mateo have new Wavier Agency Assignments. Additionally starting 2025 Imperial County will also have a new assignment. Please see the chart below for current assignments.

All impacted Waiver Agencies have been working closely together in coordination with ISCD to assure these transfers do not affect the quality of care of current beneficiaries and applicants. Furthermore, any new applications received from San Francisco and San Mateo counties shall be submitted to the newly assigned Waiver Agency.

How to submit an application for the HCBA Waiver:

  1. Refer to the table above to see which Waiver Agency serves your county/zip code. Click on the Waiver Agency name to be directed to their webpage
  2. Contact the Waiver Agency to request an application
  3. Complete the application and submit it to the Waiver Agency

​HCBA Grievances​

If you are dissatisfied with your HCBA Care Management Team, or the HCBA services you receive, you may submit a grievance in the following ways:

  • Grievances regarding the quality or receipt of HCBA services must be submitted to your Waiver Agency via email, mail, or telephone. Your Waiver Agency’s contact information can be found linked in the “HCBA Waiver Agencies” section above.
    • If you reside in: Alpine, Imperial, Inyo, Marin, Mendocino, Mono, or Napa counties, your grievance must be submitted to DHCS via email at: ISCDCompliance@dhcs.ca.gov.
  • Grievances regarding the coordination of care by your Waiver Agency must be submitted to DHCS via email at: ISCDCompliance@dhcs.ca.gov.
  • Grievances regarding your Congregate Living Health Facility must be submitted to DHCS via email at: ISCDCompliance@dhcs.ca.gov.
